However this scalpel free surgery is in fact a refinement of the old punchgraft technique. Small (1mm or smaller) punches are used to free the superficial skin around the hair root which is then popped out. Dr Rogers is the only UK hair surgeon currently offering this surgery and will shortly be scheduling dates for demonstration procedures both in London and Stratford on Avon. This will involve removing 10 FUE grafts and allowing patients to view their own follicles under the microscope! Please phone to book this in cost £100. It has definite advantages but also definite disadvantages. The advantages are the potential to use a greater amount of the possible donor area and to still have a close shaven head. The disadvantages are higher cost fewer grafts are possible per session and the very important issue of damage to the hair follicles whilst removing them. Whilst in New York Dr Rogers met Dr Rob Jones (well known FUE exponent) for lunch and discussed the finer details of the technique. Dr Jones explained the practical technicalities and showed his demonstration video. It certainly appears to be a viable technique which cannot be ignored although it is important to realise that it is still new and relatively untested. She had lost hair growth over the right forelock area which was difficult to disguise.She received only a small session of 500 grafts in case of poor scalp blood suppy to that area. After only 3 months however she is noticing significant regrowth and her 6 month post op photos will be shown as soon as we are able.Although hair restoration is primarily thought of as a treatment for male pattern baldness it also has other uses such as this eyebrow restoration disguising of facelift scars etc./P PUpdate: This patient has now returned one year after her second session (which was 750 grafts) for a total of only 1250 grafts so far although she has received a further 650 grafts which should now be sufficient. This may be because of burns and trauma genetic or cultural reasons. Since the advent of the FUE techniques experiments have been carried out to transplant body hair e.g. Chest hair to the scalp. The evidence so far does suggest that some of the characteristics of the body hair do change (length texture etc) and become more like scalp hair. This may become an additional donor source for some patients. Hair transplants can also be performed for trichotillomania facelift and browlift camouflage or simply bad scarring from head injury or brain surgery. Feminisation of the hairline is increasingly used for transexual or transgender patients. His first patients are now returning with early hair regrowth which has confirmed to Dr Rogers personally that this is a viable technique. There are still limitations at the moment mainly speed of graft extraction but there are a number of surgeons who are leading research into this. Dr Rogers has invested in some of the new equipment required that his American colleagues are using and is now achieving good results with 250 graft sessions with the expectation of 500 graft sessions soon. Dr Rogers mainly is recommending FUE as a good technique for maximising the use of the total donor area which is something that strip surgery cannot address. However strip surgery remains the Gold Standard for most patients in terms of reliability cost effectiveness and density/coverage issues. http://www.westminsterclinic.co.uk/site/news.asp 2000 graft sessions now available With the recent advances in techniques and equipment we are now able to perform 2000 follicular unit graft mega sessions on selected patients. It is important to understand that this is dependent on a high density donor area preferably with dark thick calibre hair. The patient will be a Norwood classification of 4A or higher. It will take on average seven hours to perform although the patient will have a break in between hair removal and reimplantation. The introductory price is £4250 ($7650)which equates to £2 per graft. http://www.westminsterclinic.co.uk/site/news.asp Hair Multiplication Trial Update We are looking for a patient Norwood 56 or 7 to take part in a small hair multiplication study. It will consist of 25 unmodified FUE grafts placed in a 1cm square on the left vertex compared to 25 multiplied FUE type grafts placed similarly on the right vertex. The growth of these will then be compared at 3 monthly intervals. If you wish to take part please let us know and well discuss the details more fully. However the decision on which method of surgery will be best suited to individual patients must lie with discussion at consultation. http://www.westminsterclinic.co.uk/site/news.asp Body Hair Transplant via FUE UK First Dr Rogers has just performed chest hair transplantation into a central chest scar (from an open heart operation). Follicular Unit Extraction was used to remove existing chest hair from the pectoral areas into the wide scar. This provides a perfect hair match rather than using scalp hair to camouflage the hairless scar. This would not have been possible without the development of FUE. Follow up photos as soon as hair regrowth is established. http://www.westminsterclinic.co.uk/site/news.asp Scar prevention with Isolagen Dr Rogers has been researching different techniques to minimise scarring in the minority of patients who are not naturally good healers i.e. Their skin is unusually lax (Ehlers Danlos type) and does not form the usual strong hard to see scars (see gallery). In combination with his routine careful attention to detail meticulous surgical technique and long lasting subdermal stitches most patients should not fear the minor surgery involved in modern follicular hair transplantation. However in those that do have concerns Dr Rogers is planning to conduct trials in association with Isologen to improve the cosmesis of these. Isologen is a company which muliplies and activates a patients own fibroblasts which have been harvested from a skin biopsy sample. Fibroblasts are the cells involved in collagen production essential to provide the scaffolding to the skin. Initially used in improving facial wrinkling and sagging it has had proven benefits in scars from burns etc. It occurred to Dr Rogers that these activated fibroblasts could be used to ensure strong scar formation unlike the weak scars that stretch in these particular patients. Dr Rogers is therefore looking for patients with this type of healing reaction to take part in a controlled trial. http://www.westminsterclinic.co.uk/site/news.asp Hair Transplantation in the UK dying out? Hair transplantation is a dying art in the UK. There are only a handful of full time Hair surgeons left in the UK including Dr Rogers and there may be no more. Recent Healthcare Commission regulations are making it very difficult for doctors to train in hair restoration because under the new regulations all future hair surgeons must first be FRCS Plastic surgeons before training to do hair. It is very unlikely that such surgeons will commit to full time hair restoration which is generally agreed to be essential to keep up to date in this rapidly evolving speciality. Secondly the new regulations are reclassifying small independent clinics such as Rogers Medical as Acute Hospitals. This means they are classified the same as your local NHS district hospital with an increased requirement for paperwork more expensive equipment staffing and inspection charges etc. These requirements are already having a knock on effect the prices that patients pay will be increased. Although in general these regulations are beneficial they were never written with small clinics in mind and are inappropriately applied a clinic performing minor surgery such as hair transplantation is not the same as your local District General yet both are Acute Hospitals. Experienced hair doctors such as Dr Rogers will not be affected by the changes but even so Dr Rogers is still lobbying the HealthCare Commission to adopt a commonsense approach before this highly specialised surgery dies out. http://www.westminsterclinic.co.uk/site/news.asp Hair Loss Consultation fees costs and prices Rogers Medical has previously offered free consultations to discuss hair loss options. Unfortunately as hair transplantation has evolved more time is required to extract the hair and meticulously place thousands of grafts. As Dr Rogers believes that prospective patients must always see the surgeon not a salesperson it means that his time is ever more precious. Dr Rogers is also very hands on during surgery even though these longer cases require him to delegate some of the procedure to nursing staff and technicians. Therefore we will shortly start charging a flat fee of £50 for a personal consultation with Dr Rogers which usually lasts around half an hour. This will be refunded from the cost of any procedure undertaken. Review consultations at six months or the removal of stitches will as always be free. As some of the options Dr Rogers will discuss involve either suggesting that patients wait before surgery or that they are unsuitable then it is in the interests of the patient to get this information before committing themselves to any further financial outlay. http://www.westminsterclinic.co.uk/site/news.asp Suitable Body Hair Transplant ( BHT ) candidates Although there has been a lot of interest in body hair transplantation using FUE (ie from chest back shoulders etc) it is far from an established treatment. Obviously the hair itself is usually quite different although sometimes there may be a surprisingly close match. There are differences in texture and hair length although this anecdotally does improve with a number of hair cycles. http://www.westminsterclinic.co.uk/site/news.asp Hair Transplant Costs and Clinic Policy Our policy is to normally perform hair restoration surgery on only one patient per day whether it is only 750 grafts or 2000 plus. We feel that this is the correct approach if the patient or a staff member is delayed we know that we dont need to rush. In addition rushing local anaesthetic injections can make them uncomfortable when normally you should be hardly aware of them. On the day of surgery we recommend a good breakfast it will make you feel more comfortable during the procedure. Patients usually arrive at 0915 for a another consultation with Dr Rogers photos and a BP check with one of the nurses. After taking the grafts the patient then breaks for lunch before returning for graft reimplantation. We normally finish by mid afternoon. We are proud of this personalised holistic service. One element of this is the number of grafts performed in one session. However to make this possible the patient has to be a good candidate. Some of these factors are: High donor density that is a high number of hair follicles per centimetre squared. This varies widely from patient to patient but as a rule of thumb if you cant see through to the scalp when your hair is cut to clippers 4 level then you have high density. Medium to thick hair calibre (coarseness). Thicker hair is generally easier and quicker to cut under microscopic dissection than fine hair. This is also true for hair colour darker is better. Grey or white hair is unsuitable for high graft numbers. The recipient area should be bald or sparse and shaven. Although moderate loss can be thickened up by hair transplantation the hair loss should be stabilised first by DHT blockers such as Propecia. Patients with moderate loss are also advised that shaving the area will give a better result than leaving it long although for understandable reasons if patients need to keep their hair long it is still possible to transplant it but not as many grafts in one session. Patients should not have had previous hair transplantation (or only a small session) as ideally large sessions are only possible on virgin scalps. Finally Dr Rogers will advise you of the numbers of grafts possible at consultation. http://www.westminsterclinic.co.uk/site/news.asp FUE shaving the donor area After 16 months experience with FUE it is important for patients to realise that there is a compromise which must be made between achieving 500 grafts or shaving a smaller narrower band of hair to hide the donor area where the grafts have been removed. Some patients with low density hair require large areas to be shaved in order to get the required number of grafts. They are one of the many such devices devised however they havent really caught in most clinics. They combine the making of the pin prick incision with insertion of the hair graft. Unfortunately they were initially designed with thick straight Asian hair in mind ( by Dr Kim of Korea ) and so they dont necessarily work as well with Caucasian hair. They also need additional staff to implement the use of it. This ensures that the medication is appropriate for that patient.We also use a reputable pharmacy to dispense prescriptions individually to fulfill our legal requirements.In contrast a recent investigation by the MHRA ( Medicines and Healthcare products Regulatory Agency ) discovered that approximately one third of all internet precriptions were in fact fake. Not only is that a waste of hard earned money it denies the patient of potential benefit. In hair loss terms taking fake drugs means losing more hair but believing that they simply dont work for you. Trichologists ( hair scientists ) have studied such trichological problems. Below is a short guide to tricology and the top five treatments:/P PSTRONGNatural and Alternative/STRONG/P PIn the search for male and female hair restoration of thinning hair many people do turn to non orthodox or non medical hair loss treatments first. These will include various herbal or aromatherapy options. However although used for centuries there is little concrete evidence of their effectiveness. Only two have some degree of efficacy Fenugreek and Saw Palmetto. However at best these are much less effective than the DHT inhibitors./P PSTRONGNutritional Deficit/STRONG/P PIn some women (and occasionally men) vitamin or mineral deficiencies can cause hair problems. Most commonly iron deficiency causes thinning of the hair texture giving a female pattern hairloss or balding appearance i.e. An oval area of scalp see through over the top (vertex) part of the head. However this symptom or appearance can be indicative of female pattern balding (aka Ludwig pattern) which is believed to be genetic although not androgenetic as it also appears to occur in men. Fortunately in females this does not cause complete baldness but it can look very thin./P PA lack of nutrients definitely will not cause an androgenic pattern ( male pattern baldness or MPB ) of hair loss in men or women. Typically this is frontal thinning or recession of the bald temples. The best medical treatments for this are:/P PSTRONGPropecia ( Finasteride ) and Avodart (Dutasteride )/STRONG /P Pwhich are dihydrotestosterone blockers. They block the conversion of testosterone into DHT which is the hormone resposnible for MPB and prostate enlargement. DHT also has an effect on sebum production in the skin which has a role in seborrheic dermatitis ( dandruff ). In 80% of men they can give real hair growth or even regrow lost hair follicles sometimes dramatically. It is important to understand that these follicle bulbs are not dead but dormant. If in the future new techniques can reawaken them that hair will grow again./P PSTRONGHair Surgery/STRONG/P PBest known of these techniques is hair transplantation although both scalp reduction and even flap surgery or balloon expansion has been successful. Hair transplants have been performed since the 1930s originally by Dr Okuda from Japan./P PAlthough punch graft transplantingandnbsp;worked punchgrafts did not give a good cosmetic appearance and were superseded by mini and micro grafting in the 1980s and then modern micrografts and follicular units in the 1990s./P PThis is the state of the art today mainly follicular grafts mixed in with multiple unit grafts MUGs./P PSTRONGHair Pieces/STRONG/P PIts fair to say they can look very good. However this costs both time and money to achieve. Such proof would probably take that long./P PHowever there has been success in growing hair in volunteers but the hair hasnt looked normal. It has been strange curly kinky hair growing at odd angles out of the scalp. /P PIn the meantime hair loss sufferers need to maintain their hair until then with todays techniques./P Pandnbsp;/P http://www.westminsterclinic.co.uk/site/news.asp FUE Megasessions 1000 grafts now available PFollicular unit extraction is a popular new technique of hair transplantation. However it isnt always suitable for everybody. One of the disadvantages it has is the slowness of extraction compared to the Gold Standard of microscopic strip dissection. Three methods used to get around this are:/P PPerforming a long session 1012 hours/P PSpeeding the extraction rate/P PPerforming two sessions back to back (two days in a row)/P PAt Rogers Medical we have been working on all these ideas and think we may be able to shortly increase our graft numbers. However experience has shown that it is still a variable technique. On selected clients we have achieved 700 grafts. With additional staff we think we can now achieve 1000. The latest treatment is PRP or Protein rich Plasma which is the fluid flowing through blood vessels carrying the blood cells along. He called it the slanticle. It is something Dr Rogers has performed for a long time but is a small detail that only someone as skilled as Dr Shapiro might notice. There has been debate and research about angulation of the grafts for some time coronal or sagittal. But of course this only takes account of 2 directions.
